The Achilles tendon presents unique challenges for ESWT: its midportion watershed zone has limited vascularity, its insertion absorbs both tensile and compressive forces, and its size demands precise energy targeting. Understanding this anatomy and pathophysiology is essential for selecting the correct modality (focused vs. radial), calibrating treatment parameters, and integrating ESWT with loading-based rehabilitation for durable clinical outcomes.
You’ll discover how tendon stem cells, neurovascular changes, and collagen remodeling all play a role in the failed healing response—and why ESWT can stimulate regeneration where conventional treatments often fall short. Twelve months after high-energy focused ESWT, 85% of patients with chronic midportion Achilles tendinopathy that had resisted conservative care rated their result good or excellent — compared with about 26% of matched controls. The module dissects this landmark study alongside four other clinical trials.
